Be prepared by wearing a medical ID bracelet with secure information on your medications, medical conditions and personal contacts. A medical ID can quickly provide this information to emergency medical techs should you suffer a mishap while riding. Many of us use a bracelet or a “sleeve” that fits on a watchband, sold by MyID.com.

 

If you don’t want to purchase a bracelet, consider completing this 

Emergency Medical Info Form and keeping it in your pocket or saddle bag when riding.

Ride Leader Considerations  – Suggestions to club ride leaders that emphasize the safety, comfort and enjoyment during group rides. Currently being edited.

 

Release of Liability form Ride leaders should carry two copies for guests to sign. (One for the guest, one for the club.)

 

The Siskiyou Velo Accident Report Form is for ride leaders. It needs to be completed should a mishap or accident occur and the injured party wishes to initiate an insurance claim. Contact the SV president or other board member for guidance on submitting this form.

 

The First Report of Accident Form is required by the club’s insurance company if an injured cyclist intends to file a claim. The SV President or other board member can assist a ride leader in completing this form.
