The documents and suggestions below provide guidance to our club’s volunteer bike ride leaders and cyclists participating in club-sponsored rides. They will help you understand what is expected of you as a ride leader or participating rider who participates in club-sponsored rides and other activities; suggestions to improve your safety and satisfaction while riding; etc.

Release of Liability form (7/2/2023) — to be signed upon joining the Siskiyou Velo club. One-time guests on a club-sponsored ride are also asked to sign the release of liability.
Emergency Medical Information Form — always carry contact and medical/insurance information with you, either on a paper form or electronically.
